Student-Athlete Registration for the 2026-2027 School Year is Now Open!
We are excited to announce that Student-Athlete Registration is now open for the 2026-27 school year through Final Forms(Opens in a new window)! All student-athletes and students involved in school-sponsored activities must have their Final Forms updated and completed before participating.
Returning Students
All returning High School student-athletes and members of student organizations that use Final Forms must log in and update their forms for the upcoming school year.
Both Parent and Student must complete the Final Forms sign-off process to be considered fully registered.

New Students (Incoming Freshmen or Transfers)
All athletic forms for new students are completed online through Final Forms.
To register a new student, a parent or guardian must first create an account and invite their child. There are two ways to do this:
- If you already have a Final Forms account from a previously registered child, simply log in and add your new student.

Submitting a Physical
The only document you need to submit is the two-page physical form signed by a licensed physician.
You may submit physicals in any of the following ways:
- Upload directly to Final Forms
- Email a scan or photo(s) to Mr. Wolf: [email protected]
- Drop off at the Athletic Office during open school hours
Please do NOT turn in physicals to your coach.
